Position Summary:
Florida Atlantic University is seeking a Social Worker - Boca Raton, FL.
The Social Worker will function as an integral member of an inter-professional healthcare team, providing services and supportive care at the Louis and Anne Green Memory and Wellness Center, located at the Christine E. Lynn College of Nursing's Boca Raton and Davie campuses. Under the supervision of the Director of the Memory and Wellness Center and the Administration for Community Living Grant Project Manager, the social worker plans and coordinate direct services for individuals affected by dementia.
Summary of Responsibilities:
Conduct home visits to assess ongoing care coordination needs of individuals living alone with dementia, including unmet needs, social isolation, and home safety/risk.
Maintain accurate, well-written documentation, progress notes, and required reports.
Develop individualized care plans in collaboration with interprofessional team members using evidence-based interventions
Identify existing or potential caregivers to support individuals living alone and facilitate enrollment into the REACH Community Coaching program.
Attend grant project meetings with interprofessional team members to support coordination and integration of services.
Identify and assist with development of new grant opportunities to ensure sustainability of services and supports.
Assist in developing and distributing marketing materials, brochures, and flyers related to the Memory and Wellness Center and grant services.
Share internal and external resources with community partners and families to support individuals with or at risk for dementia. Facilitate and expand family caregiver support groups.
Provide educational lectures and presentations to patients, caregivers, and the broader community.
Participate in quality improvement initiatives to evaluate and enhance program effectiveness.
Stay current with best practices, policies, and resources relevant to dementia care and social work in a clinical setting.
Perform other job-related duties as assigned.

Minimum Qualifications:
Master's degree from an accredited institution in an appropriate area of specialization; or bachelor's degree from an accredited institution in an appropriate area of specialization and two years of appropriate experience required.Preferred Qualifications/Skills
